The information we collect

Depending on how you engage with us, we may collect:

  • Personal details — your name, date of birth, contact details, and emergency contacts.
  • NDIS information — your NDIS number, plan details, funding arrangements, goals, and the names of others involved in your support (such as nominees, plan managers, or support coordinators).
  • Health and sensitive information — information about your disability, health, care needs, and supports, which we treat as sensitive information under the Privacy Act.
  • Service records — notes, communications, feedback, and records relating to the supports we provide.
  • Website information — if you contact us through this website, the details you choose to provide in your message.

How we collect it

Wherever possible, we collect information directly from you. With your consent, we may also collect information from people you have authorised to act or speak on your behalf — for example, a family member, nominee, guardian, plan manager, support coordinator, or another provider involved in your supports.

Why we collect and use it

We collect and use your information to:

  • provide, plan, and coordinate supports that are right for you;
  • communicate with you and the people involved in your support;
  • keep accurate service and safety records;
  • respond to enquiries, feedback, and complaints; and
  • meet our legal, funding, and NDIS-related obligations.

When we disclose it

We only share your information where it is necessary to deliver your supports or where we are required or authorised by law. This may include sharing, with your consent, relevant information with:

  • our support workers and team members involved in your care;
  • people you have authorised, such as nominees, family members, plan managers, or support coordinators;
  • the National Disability Insurance Agency (NDIA) and relevant NDIS bodies; and
  • health practitioners or other providers, where this is needed for your safety or supports.

We do not sell your personal information, and we do not disclose it to overseas recipients.

How we keep it secure

We take reasonable steps to protect your information from misuse, loss, and unauthorised access, modification, or disclosure. This includes limiting access to those who need it to support you, using secure systems and passwords, and securely storing or destroying information when it is no longer required and we are no longer legally obliged to keep it.

Accessing and correcting your information

You have the right to ask for a copy of the personal information we hold about you, and to ask us to correct it if it is inaccurate, out of date, or incomplete. To make a request, please contact us at contact@safestarsupports.com.au. We will respond within a reasonable time and will let you know if there is any reason we are unable to provide access.
